It had been gnawing at him a while. Now days after her operation, still he had not visited her in the hospital. He decided to write a goodbye letter, for he could not say it face to face, especially now.
Sitting in a blank space he tried to think of a way to tell her he simply couldn’t be with her. He thought they’d be together forever, but now had unending doubts. In her condition, this wasn’t his dream of happiness any longer.
Writing three words, unsigned, was his choice. It was his fault and he couldn’t face the guilt.

Well written three lines. However, I can see your character writing a lot more notes like this as he goes through life.
A really dramatic ending to this tale would be if she read the note and gave up the ghost.
